Adverse Drug Event Risk Assessment
Do you take five or more medicines?
Do you have three or more medical conditions for which you are receiving medicinal treatment, e.g. high blood pressure, diabetes, heart disease?
Do you have more than one provider that prescribes your medicines?
Are your medicines obtained from more than one pharmacy?
A “YES” answer to the above questions could indicate an increased risk for experiencing an adverse drug event.
To help prevent an adverse drug event, consider obtaining a comprehensive review of your medicines and supplements with a medicine expert, such as a Medication Therapy Management pharmacist. Additionally, a certified geriatric pharmacist is specially trained to address medication problems in older adults.
